Fraser is looking for a Case Manager to serveindividuals withdevelopmental disabilitiesinHennepin, Ramsey and Dakota counties. The Case Manager serves as an advocate to Fraser clients and connects clients to resources and services within the community. The ideal candidate will demonstrate compassion for clients, commitment to success and value relationships. We seek an individual who is very well organized, has experience with individuals with developmental disabilities and experience documenting and case noting.

The Case Manager provides client-centered case management services to individuals receiving DD waiver services. The Case Manager may also support individuals with developmental disabilities who are not on a waiver or who utilize Consumer Directed Community Supports (CDCS). Case Managers typically have about 45-50 clients on their caseloads and meet with clients three times per year or once every four months. The Case Manager is responsible for care coordination, monitoring health and safety needs of clients, monitoring services to ensure they are effectively meeting the client's needs and intervening as necessary. Caseloads are typically a mix of children and adults.

Location and Schedule:

Our Case Managers work Monday - Friday during standard business hours. This time is split 50% in-office at Fraser Bloomington (1801 American Blvd E, Bloomington MN) and 50% virtual office. We provide services in Hennepin, Ramsey, and Dakota counties, with each Case Manager assigned to one of these three counties.

  • At least a bachelor degree in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of persons with developmental disabilities or related conditions required.
  • If degree is in social work, social work licensure is required.
  • One year of experience in the education or treatment of persons with developmental disabilities or related conditions required (QDDP qualified).
  • One to two years of experience working in service coordination or case management preferred.
  • Valid Minnesota Driver's License with acceptable record.
  • Ability to pass DHS background study.
  • Commitment to promoting diversity, multiculturalism, and inclusion with focus on culturally responsive practice, internal self-awareness and reflection.
  • Effective communication in the English language, including verbal and written communication with both internal and external contacts.
  • Ability to perform basic mathematical skills, including accurate billing.
  • Ability to understand and implement all related regulations, policies and procedures.
  • Ability to recognize problems, establish facts, and draw valid conclusions.
  • Computer proficiency using Word and Excel with proficiency in keyboarding skills.
  • Excellent organizational skills and attention to detail.
  • Basic mathematical skills, including the ability to add, subtract, multiply, divide and calculate percentages.
